The Macomb County Health Department now offers the ability to schedule appointments for most immunizations online. If you wish to make an appointment for an immunization, please see below.

Waiver education

Waiver education is only available by appointment through the School Immunization Program. Please call 586-466-6840 to schedule an appointment.

Forms

What to bring to your appointment:
•    A photo identification card.
•    All insurance cards (if applicable).
•    All Immunization Records-especially if done outside of Michigan
•    You can save time by printing and completing your Immunization Forms before your visit. Printed copies will also be available at the clinic. For children, all forms must be completed by a parent/legal guardian.

Please Note:

  • Children under age 18 MUST be accompanied by a parent/legal guardian. If a parent/legal guardian is unable to accompany the child, then another adult must be authorized to accompany the child. An “Authorization for Immunization for Minor” letter will need to be completed and brought with the child to their visit. This authorization letter can be found above.
  • If you have recently been exposed to or tested positive for an infectious disease (i.e. Covid-19, flu, measles, etc), then please reschedule your appointment or call one of our locations for additional guidance.

Appointments preferred. Walk-ins accepted as time allows. Clinics are also closed in the mornings on the 3rd Friday each month. 

Monday, Tuesday, Thursday, and Friday: 8:30 a.m. – 4:30 p.m.
Wednesday: 8:30 a.m. – 6:00 p.m. 

TB skin tests cannot be placed on Wednesdays after 4:30pm or at any time on Thursdays. Holidays may also affect the days and times in which TB skin tests can be placed. Please note that clients who have TB tests placed will need to return to have the test read by a nurse 48-72 hours after placement.

Cost of vaccines

  • Macomb County Health Department can bill some commercial insurances for immunization services.  Please contact one of our health centers to check if the Macomb County Health Department is a provider for your commercial insurance. Final determination of eligibility/benefits must occur at time of service.
  • Macomb County Health Department does participate in the Vaccines for Children Program (VFC).  Participants of this program who are unable to afford the administration fee should talk with one of the clinic nurses.
  • There is a vaccine administration charge of $10 for each vaccine given. In addition, vaccine fees will be charged to individuals who are not eligible for federal and/or state vaccine programs.  
  • Payment by cash, check or credit card. An enhanced access fee is applied for credit card usage based on the amount of service being charged. We cannot accept Health Savings Account (HSA) or debit card only transactions.
  • Medicaid/Medicare Part B will be billed for approved vaccines. At this time, we cannot bill for vaccines covered under Medicare Part D.
  • Adults may be eligible to receive certain vaccines at a reduced cost. Talk with your immunization nurse to see if you qualify.

Vaccine information statements (VIS)

In Michigan, it is important that vaccine recipients, their parent, or their legal representatives be given the Michigan versions of the VIS because the include information about the Michigan Care Improvement Registry (MCIR).
