Forms
Save time. Please print and complete a registration form for each individual who is to receive a flu vaccination.
Child forms must be completed and signed by a parent or legal guardian. A parent or guardian MUST be present for each child. Legal guardians MUST present proof of guardianship.

- Medicare, Medicaid, and some commercial insurances are accepted
- Cost for clients wishing to self-pay = $20
- Clients younger than 19 years of age may be eligible to receive the flu vaccine at a lower cost through the Vaccines For Children (VFC) program. Details about this program can be found here

- Clients whose insurance covers flu vaccination, but plan is not listed above, must contact their insurance providers for guidance if they wish to seek flu vaccination reimbursement.
- All clients have the option to self-pay $20 for flu vaccination.
What to bring
At the time of the appointment, please bring:
- Photo ID
- All insurance cards including Medicaid, Medicare and any other supplemental insurances.
- Completed registration forms (if possible)
- Note: Blank forms will be available at the vaccine site if you do not have access to a printer.
