1. Request for Hearing on a Motion
This is the local form that is used to place a case on the motion docket for hearing by a judge on a motion day. It can also be used as the notice of hearing and proof of service. It is called a Request for Hearing on a Motion. For an Adobe PDF version, click here.
Prior to filing your request for hearing, please take a moment to review the available motion dates and times on the Judge's Motion Schedule found here.
2. Family Division Confidential Asset and Liability Disclosure sheet
The Family Division requires in the initial order that all parties complete these sheets and bring them to the first conference. For an Adobe PDF version, click here.
3. Facilitation Order
This is the order which is used to refer cases to facilitation, a form of voluntary alternative dispute resolution utilized by the Court on a regular basis. For an Adobe PDF version, click here.
4. Miscellaneous Order

5. Applications for Attorneys to Represent Indigent Persons (Domestic and Juvenile)
Application forms for attorneys wishing to be added to the Court Appointed Counsel List for Domestic and Juvenile cases are available by clicking on the following links:

These forms may be completed on your computer. Alternatively, you may print out the forms and fill them out by hand.
The Court accepts applications to be placed on the Court Appointed Counsel List or to be upgraded to a different List throughout the year. The Courts' Indigent Panel Committee meets quarterly to review applications. The process is governed by Local Administrative Order 2020-06. Please fill out the application(s) for any List you are interested in and return the completed application to Case Management, 6th Floor, Macomb County Courthouse, 40 N. Main, Mt. Clemens, MI 48043. Applications may also be emailed to shelley.lemire@macombgov.org.

6. Appointed Counsel request for payment forms
Orders from District Court for Reimbursement of Defense Costs The Circuit Court has distributed a new Order for Reimbursement of Defense Costs to the District Courts in Macomb County, for use where a felony case is either resolved or dismissed in their Court. This Order establishes whether defendant has been ordered to reimburse defense costs. If the District Court issues an Order for Reimbursement of Defense Costs, this Order must be attached as a .PDF to your Request for Payment.
Effective February 1, 2016, only court approved electronic Requests for Payment from our website will be accepted. We recommend that the following links are used. Forms do change and we do not want to delay payment for services rendered. Once the form is electronically submitted, a copy of your request will be sent to your email address in a .pdf format for your records. Separate attachments may be sent to: cacbillsonly@macombgov.org.

7. Indigent Attorney fee schedule
This is the schedule that controls, as a general rule, payment of attorneys who are appointed to represent indigent defendants in the Macomb County Circuit Court. Click here.

8. Case Evaluator application
This document is the application which attorneys who are interested in becoming case evaluators under MCR 2.403 should complete and file with the Case Evaluation/ADR Clerks. 9. ADR Order
This is the order which is used to refer cases to Alternative Dispute Resolution and appoint a mediator. For a PDF version, click here.
10. ADR Mediator application
This document is the application that parties who are interested in becoming ADR mediators should complete and file with the Case Evaluation/ADR Clerks. For a PDF version, click here.
11. Petition for Appointment of Counsel (Juvenile)
This document is used for parties that are indigent and are required by law to have legal counsel represent them. This form must be submitted to Judicial Aide. For a PDF version, click here.
12. Fee Waiver request
This document is used to request waiver of filing fees. Complete the form and file it with the Macomb County Clerks Office. For a PDF version, click here.
13. Request for payment of Court Appointed Interpreter fees
This document is the billing form to be used by foreign language and American Sign Language (ASL) interpreters. This form must be completed online and will be automatically transmitted to Judicial Aide for processing.
