COVID-19 vaccination appointment scheduler

The Macomb County Health Department has two locations where clients age six months and older can receive a COVID-19 vaccination.

General information

  • Appointments preferred. Walk‐ins accepted based on vaccine availability.
  • Anyone age six months and older can receive a COVID-19 vaccination.
  • Primary series and booster vaccination appointments can be scheduled.
  • Two vaccine sites are available. See location information below for details on days and hours of operation.
  • Vaccines offered at clinic locations include: Pfizer, Moderna, and Novavax.

Prepare for your vaccine appointment

What to bring: 

  • A photo identification card.
  • A print out of the COVID-19 Vaccination Form (based on client’s age):
  • Your COVID-19 Vaccination Record Card (if returning to complete your Primary Series or receive a Booster).

 

Please be aware:

  • Clients six months through age 17 must be accompanied by a parent or legal guardian.
  • Do not bring children other than those who will be vaccinated to the appointment.
  • Only the person being vaccinated will be allowed in the building to maintain safe social distancing. Exceptions will be made for a family member or caregiver to accompany someone who needs assistance getting into the building.
  • Face masks are required to be worn inside the building.
  • Reschedule your appointment if you are experiencing any symptoms of COVID‐19 (such as fever, cough, chills, etc.).
  • There is no cost for vaccination and your insurance will not be billed.
