- Appointments preferred. Walk‐ins accepted based on vaccine availability.
- Anyone age six months and older can receive a COVID-19 vaccination.
- Primary series and booster vaccination appointments can be scheduled.
- Two vaccine sites are available. See location information below for details on days and hours of operation.
- Vaccines offered at clinic locations include: Pfizer, Moderna, and Novavax.

Prepare for your vaccine appointment
What to bring:
- A photo identification card.
- A print out of the COVID-19 Vaccination Form (based on client’s age):
- If six months through 11 years old: Child COVID-19 Vaccination Form
- If Age 12 or Older: Standard COVID-19 Vaccination Form
- NOTE: Printed forms are available at the clinic if you do not have access to a printer.
- Your COVID-19 Vaccination Record Card (if returning to complete your Primary Series or receive a Booster).
Please be aware:
- Clients six months through age 17 must be accompanied by a parent or legal guardian.
- Do not bring children other than those who will be vaccinated to the appointment.
- Only the person being vaccinated will be allowed in the building to maintain safe social distancing. Exceptions will be made for a family member or caregiver to accompany someone who needs assistance getting into the building.
- Face masks are required to be worn inside the building.
- Reschedule your appointment if you are experiencing any symptoms of COVID‐19 (such as fever, cough, chills, etc.).
- There is no cost for vaccination and your insurance will not be billed.
