The annual update ensures we can reach you and bill the correct insurance. You may also opt in to the forthcoming CSHCS quarterly email newsletter.
In addition to the required annual update, we ask enrolled families to complete a short survey about CSHCS services. Tell us how we're doing and how CSHCS can improve! A QR code on the annual update will take you to this survey. Your responses help guide future program activities.
